Welcome to UnitedHealthcare Community & State. We're the part of UnitedHealthcare brand that's responsible for 25 Community Plans in 24 states and the District of Columbia.

Our mission is to help people live healthier lives by ensuring access to high quality health care services for the underserved. We accomplish that mission through innovative strategies like our Winning Priorities approach, and through innovative programs such as the Personal Care Model, Healthy First Steps, and effective disease management.

We work hard at being a good partner for states, providers and members by constantly trying to be easier and simpler to do business with.

And we're committed to Medicaid and CHIP, to the many communities in which we work, to a local presence and local service, and to the understanding of local culture and its relation to disease and disease management.

Medicaid Plans Overview

Medicaid is a program for people who can't afford to pay for medical care. It is offered by states to their residents who are United States citizens or qualified immigrants. To get Medicaid, you must be a pregnant woman, a child, a member of a low-income family, or must be aged, blind or disabled. A few states also cover single, healthy adults. You also must meet state income standards, and certain other requirements.

To find out about Medicaid in your state, please visit the Center for Medicare & Medicaid Services Web site.

Some people qualify for Medicare and Medicaid coverage at the same time – these people are known by the term "dual eligible" and there are specific health plans for them.

Children’s Health Insurance Programs (CHIP) Overview

You can get affordable health care for your children through UnitedHealthcare-administered CHIP programs in many states. Depending on where you live, your children can have insurance to cover doctor visits, hospital care (including emergency room care), immunizations, routine physical exams and prescription drug coverage.

We’re a great health care partner for your kids. We insure more children than anyone else, and we’ve been doing it for more than 20 years. No one has a larger list of pediatricians and children’s hospitals. And we’re a leader in working with community organization to help uninsured children become insured, and to help insured kids get the best medical care possible.
