How To Fill Out Forms

Health care professionals and the federal government take your health information privacy seriously ... You should too. Some of the information needed will vary depending on what you are filling out, but much will be the same.

Whether you're applying for a certain job, seeing a new doctor, getting insurance even buying personal training sessions, you probably will be asked to fill out a health history form. In today's information-based society, these forms give potential employers and health-care providers a glimpse into your personal health history. The rule to remember when filling one out is to be honest.

Keep your information safe

Personal information you give to your doctor is shared with insurance companies, pharmacies, researchers, and employers based on specific regulations. The privacy of your health records is protected by federal law (the Health Insurance Portability and Accountability Act, also known as HIPAA), which:

  • Defines your rights over your health information
  • Sets rules and limits on who is allowed to receive and/or see your health information
  • HIPAA protects your health information when it is held by most health care providers, health insurers, and other organizations operating on behalf of your health care provider or health plan.
